As a "thank you " to our referring active patients and their friends we offer a $30 Visa Gift Card to BOTH parties when a new referral becomes a patient of the practice. An active patient is defined as one who has received billable services at Brink Street Dental, Ltd, 284 Memorial Ct, Unit C, Crystal Lake, IL for hygiene or restorative procedures during the 5 year period prior to the referral.

A referred patient becomes a patient of the practice after receiving either hygiene or restorative services. Only the first individual from any one household and/or nuclear family referred in by an active patient is eligible for a gift card once they become a patient of the practice.
